Hospital Bedside Support – Be comforted knowing your patient navigator is communicating across the healthcare team to ensure there is a safe and clear plan for your care both during your hospital stay and post discharge. Having a patient navigator at your hospital bedside not only provides compassionate support but fills a critical role coordinating with your entire healthcare team.

Home Visits – While technology advances in video conferencing have made remote homecare visits possible, experiences of an in-person meeting allow for much richer and deeper personal connections. Home visits provide opportunities to establish client / navigator trust which leads to a solid foundation for all services we provide on your behalf.
Prepare for and Attend.

Medical Appointments – Most patients don’t recall about half of what the doctors tell us. Many factors contribute to this, such as limited time with your doctor, understanding your lab testing results, and feeling anxious about a medical exam. Rely on your navigator to help you organize your questions and concerns prior to your medical exam and to take notes during the exam.

Arrange for Medical Transportation – Don’t let lack of transportation prevent you from attending your healthcare appointments. We can help you to find services to get you where you need to go.

Medical Bill Discrepancies & Insurance Claims Review – Be sure you are billed appropriately. We review your statement to match your bills to your insurance explanation of benefits and clear up discrepancies.

Prepare Your Emergency Medical Contacts and Critical Medical History Information – Being prepared for a medical emergency always is essential. We create a document for you that captures the most essential aspects of your medical information, including your primary care provider, your emergency contacts, your insurance plan details, current medications and dosages, and allergies.

Patient Navigation Fees

An initial consultation by telephone or video conference for new clients is free. The consultation will include a discussion on your interests and needs for patient navigation services and decide together if we are best suited to meet your needs.

RETAINER FEE / CONTRACT

Should you decide to proceed with MD Patient Navigation after the initial consultation, a contract between MD Patient Navigation and the client describing the services will be delivered and presented for both parties to sign.

Once payment for the retainer fee is received and the contract fully executed, work will begin. The retainer fee will be applied on an hourly basis as services are delivered.

PURCHASE OF ADDITIONAL HOURS

Clients may purchase additional blocks of hours in advance or opt to pay as they go on an hourly basis for services.

BILLING

Monthly reports will be prepared showing an itemization of the hours spent towards services delivered and payments received / outstanding.

PAYMENT INFORMATION

Payments by cash and credit card are accepted.

INSURANCE

Patient Navigation services are not typically covered by insurance plans. Most insurance plans do not cover the cost of independent patient navigation services.

While many hospitals and some insurance companies offer patient navigation services at no cost, these services are typically limited. In contrast, independent patient navigators are hired directly by patients and their families and deliver a full spectrum of advocacy services whether it be a billing complaint or any other services in or out of a hospital setting.

Many hospitals offer patient navigation services upon request. However, these resources are routinely located in the hospital’s risk management or legal department and are often very limited in number.

Typically, they focus on resolving patient complaints received while they are hospitalized and to ensure hospital policies preserving patients’ rights and adherence to safety protocols by the hospital staff.

Some insurance companies offer patient navigation services to help customers with the resolution of billing complaints and to assist customers in understanding their insurance claims and benefits.
